You say the total you want to pay as you hand over the money or card, rather than leaving cash on the table. On a €37.20 bill, hand over €50 and say 'Vierzig, bitte' and you get €10 back. 'Stimmt so' means keep the change. Money left on an empty table can be taken as forgotten change rather than a tip.
Yes. German law requires prices to be quoted inclusive of VAT and service, so the menu price is what you owe and no separate service charge appears at the bottom of the bill. Trinkgeld is genuinely voluntary on top, which is why German tips are much smaller than American ones.
Usually, yes. Attended toilets in stations, department stores and restaurants expect 50 to 70 cents left in the saucer for the attendant.
