Each was derived from a different population and era — Hamwi came from diabetes dosing charts, Robinson and Miller re-fitted the Devine approach to newer data. Differences of 3-6 kg between them are normal.
No. Ideal body weight formulas use only height and sex; they cannot see muscle mass, bone density or fat distribution. A muscular person can sit above the range and be perfectly healthy.
