The Rashi Name Letter Chart lists the nine naming syllables traditionally linked to each of the twelve rashis, together with the nakshatra and pada each syllable comes from. The system is arithmetic: 27 nakshatras divided into 4 padas each give 108 syllables, and every rashi spans exactly 9 of them, so Mesha runs from Chu to A and Meena from Di to Chi. A lookup panel also tells you which rashi a name's opening syllable falls in. This is cultural reference material about a naming custom, not a prediction about anyone.

No. A rashi used for naming is the sidereal Moon sign at birth, which depends on the exact time and place of birth, while a Western sun sign is the tropical position of the Sun and depends only on the date. The two frequently differ, so a birthday-based sign chart will not give the right naming syllables.
It is a custom, not a rule, and practice varies widely by family and region. Many families give a naamkaran name from the chart and use a different everyday name; others do not follow the chart at all. If it matters in your family, confirm the rashi with the person who prepared the birth chart before choosing.
