2.5% of your net zakatable wealth, and only if that net is at or above the nisab. Net wealth means your cash, bank balances, gold, silver, trading stock, recoverable receivables and investments, minus the debts and bills genuinely due now.
Nisab is fixed in metal, not currency: 87.48 grams of gold or 612.36 grams of silver. Multiply either by today's rate per gram to get the threshold in your currency — the silver figure is far lower, which is why most scholars recommend it, as it brings more payers in for the benefit of recipients.
Not the whole loan. Most scholars allow only the instalments falling due within the year to be deducted, not the full outstanding balance, so enter that year's portion rather than the entire mortgage. Rulings on long-term debt vary between schools — check yours.
One full lunar year, or hawl — roughly 354 days, about 11 days shorter than the solar year, which is why your zakat date drifts earlier each Gregorian year. Dips in between do not matter; what counts is that you were above the nisab at the start and at the end of that year.
