Click on 'Form Builder' in the left sidebar or select any preset from the 'Quick start' bar on your Dashboard. You can choose from multiple question types (multiple choice, rating scales, confession text boxes), customize emojis, and write your questions in seconds.
Each form card has a 'Preview ↗' button and a 3-dot action menu. From the 3-dot menu, click 'Copy link' to copy the direct URL or 'QR Code' to download a printable/scannable QR code. Anyone with the link can answer your form instantly on mobile or desktop.
